In Visit 2 and 3 it can be overwhelming having to compile and present a lot of Anonymous Patient Records (APRs) and Trainee Led Case Discussions (TCDs). 
These summaries of condensed APRs and TCDs are examples of how you can plan your own records. 
This helps when it comes to explaining and answering questions about a sight test that you performed, to your examiner.
